Pharmacology is not hard. Pharmacology is disorganized. There are hundreds of drugs in First Aid, nobody tells you which ones the exam actually asks about, and every resource teaches a different subset well and the rest badly. So students do the only thing available to them: they watch everything, from everywhere, twice, and it takes three weeks.
This method removes the decision-making. You get told which drugs are high-yield, which single resource teaches each one best, and what to do after you watch. That's the whole trick — and it's why 48 hours is enough.

Four steps, run system by system. You do not move to the next system until all four are done for the current one.
Here is the actual problem nobody solves for you: no one tells you which drug is high-yield, and no one tells you where to study it from. So you either learn all 400 drugs equally — which is impossible — or you pick a resource and hope it covers the right ones well.

Open the video for that drug from the recommended resource — and nothing else. Not the Sketchy version and the Osmosis version. Not a third opinion on YouTube. The map already decided; your job is to watch.
Watch each video twice. This is not optional padding. Pharmacology is dense — mechanism, indication, side effects, interactions, all stacked into a few minutes — and one pass gets you recognition, not recall. The second pass is where it actually sticks. Two passes of one video beats one pass of two videos, every time.
After you've watched all the drugs of a system — not after each video, after the whole system — go back to the PDF and read those same drugs.
Whatever you could not retain, highlight those lines. That's it. Those highlighted lines are your memory gaps: the facts that did not survive even two viewings. That is the entire diagnostic. Everything unhighlighted is done and you never touch it again. Everything highlighted is your revision list for this system — short, specific, and earned rather than guessed.

Every drug you just watched is explained there with MCQs attached — page by page, following First Aid. So you're not testing on a random question bank, you're testing on exactly the drugs you just covered, in the order the exam's source material presents them.
This is the step that closes the loop. Watching and reading are input; the MCQs force you to apply it, which is the only thing the exam ever asks you to do.

What the 48 Hours Actually Look Like
Pick a system. Run all four steps. Close it. Move to the next. Never run two systems in parallel, and never leave a system open overnight with its MCQs undone.
- Open the PDF to one system. Read the drug list for that system only, so you know what's coming and how long it is.
- Watch every drug in that system from the mapped resource — each video twice, back to back. No other resource, no notes competing for your attention.
- Go back to the PDF and read those same drugs. Highlight every line you couldn't retain. Do not re-watch yet — the highlights are the data.
- Open usmlevault.com → First Aid Guide → that system → Pharmacology and run the MCQs for the pages you covered.
- Re-read only the highlighted lines once the MCQs are done. That's your entire revision for the system — usually a few minutes, not an hour.
- Close the system and start the next one. Antimicrobials can be its own block using the 90-minute video.
A system is done when the MCQs are done and the highlighted lines have been re-read once. Not when the videos are watched. Watching is the cheapest part of this method and the least predictive of your score.
What Not to Do
- Don't cross-check resources. Watching the Sketchy version and the Osmosis version of the same drug doubles your time and adds nothing. The map already made that decision.
- Don't watch each video once. One pass gives you recognition, not recall. The second pass is where the method lives.
- Don't read the PDF drug-by-drug as you watch. Read after the whole system — you need the gaps to reveal themselves, and they can't if the answer is on the screen next to you.
- Don't highlight everything. If half the page is yellow, you didn't watch twice. Highlights are gaps, not notes.
- Don't skip the MCQs "for now". A system without its MCQs is not finished, it's just watched. That's the exact failure mode this method exists to prevent.
- Don't try to memorize a system before moving on. Completion beats perfection. Retention comes from UWorld hitting these drugs in a vignette six weeks from now.

But it is never a new drug that gets you — it is the same twelve, arriving in a different patient. Pharm does not punish you for what you never learned. It punishes you for what you learned once and never drilled.
Mechanisms do not change between NBME forms; only the vignette around them does. And re-reading the pharm chapter re-teaches the 300 facts you already know while gliding straight past the 12 you do not — which is exactly why the third attempt at a drug goes like the first two.
