How to Actually Complete First Aid
Four readings, 4–5 days per system, the whole book in about 8 weeks — and you retain it, because each pass does a different job.
The biggest mistake people make while reading First Aid is reading it to remember it, not to understand it. Read that again. 99% of you believe you're supposed to memorise every line, because that is how you have studied for every exam so far.
That is not the way. First Aid is a dense reference — it holds everything Step 1 tests, compressed. To understand and retain it you need to go through it several times in a short span, system by system, in a different way each time. Here is exactly how.
The four readings
One book. Four passes. Each one has a completely different purpose — and that is the entire reason this works when re-reading does not.

Reading 1 — understand the page

Do not try to memorise anything on this pass. Your only job is to understand every line. When I did this myself I was constantly on YouTube and ChatGPT hunting for an explanation of the concepts I did not understand — which works, but costs hours per page.

An ELI5 pill. The page explained in plain English with an analogy, for when the paragraph refuses to click. This is the teacher First Aid never had.

MCQs on that exact page. Page-by-page questions, so you test the second you read instead of hoping it stuck. This is what primes you for UWorld — same block, same brain.
Time per system: 1–2 days for this reading. Not a week. You are understanding, not memorising — that is a much faster activity.
Readings 2 and 3 — apply it, then correct it
Real retention of First Aid does not happen while you read it. It happens the moment you start applying it. As soon as you finish understanding a system, apply it. Two routes:

Mehlman first, then UWorld. His videos are essentially First Aid questions asked out loud — a soft application layer before the real thing. Best if the system is genuinely weak.

Straight into UWorld. Blocks of 20 for that entire system, tutor mode, using
the RPSR method — Read, Pre-solve, Solve, Review. This is the route I took.

directly in First Aid — open the page, read the topic, read a couple of lines above and below it. That review
is your second and third reading of the book, and it is the step that makes First Aid stick. Skip it and you are doing half the work. Full method:

Time per system: 2–3 days for the blocks and the review.
Reading 4 — retain it

The Foundation Anki deck is First Aid, page by page. ~2,400 cards covering every high-yield line of the book, with the page number printed on every card. Running a system's cards is another complete pass of that system's First Aid — except actively asked instead of passively read.

The loop, per system — 4 to 5 days

Days 1–2 · Read. First Aid page by page on the Vault — videos, ELI5, then that page's MCQs. Understand, do not memorise.

Days 3–5 · Apply. UWorld blocks of 20 for that system (Mehlman first if it is weak), reviewing every wrong answer inside First Aid.

Alongside · Retain. Unsuspend that system's Foundation Anki cards and clear them while the pages are still fresh.
At the end of five days you have read that system's First Aid four times in four different ways — and you know how each topic is actually tested. Repeat across every system and the whole book is done in roughly 8 weeks. That is exactly what I did, and what most of my students do.
Do not do this: read all of First Aid cover to cover first and start UWorld afterwards. By the time you apply the early systems you have forgotten them, and you will conclude you are bad at Step 1 when you simply used the wrong order.
