The Exact NBME Order

Which form to take, when to take it, and what each one actually tells you โ€” 26 through 33, in the order that works.

Step 1 โ†’ everything below is for you. Forms 26 through 33.

NBMEs are the most high-yield resource we have as IMGs. They are the only thing that honestly assesses how ready you are. And there are only so many of them โ€” which is why the order matters as much as the scores.

Yes, 26 through 29 became a little more disposable once 31, 32 and 33 were released. That does not mean you should waste one. Never burn an NBME on a baseline. A form taken before you are ready tells you nothing you did not already know, and it can wreck your confidence for a fortnight.

Before you take any of them

Finish your content first. Three to four months of systems, complete. Then you start NBMEs โ€” not before. Full protocol: Taking Your First NBME.

Be at 60%, then 65%, then 70% on UWorld. Climbing through those numbers is the signal that your content base is solid enough for a form to mean something.

The three rules that apply to every single form

Always offline. No exceptions. Online NBMEs give you 50 questions in 75 minutes โ€” spacious, and it inflates your score by 4โ€“6%. You are training for the wrong exam. Why offline โ†’

Blocks of 20 questions, 30 minutes each. That is the real Step 1 rhythm, and it is the only format that predicts anything.

Finish your first pass in 23 minutes. Keep 5 to 7 minutes spare in every block for flagged questions and biostats. If you cannot do that, pacing is your problem โ€” not content. The buffer strategy โ†’

Four groups, not eight separate events. Each group has something you must finish before you open it, and each one hands you the syllabus for the next.

Your first one. Always. The easiest form there is, and that is exactly why it goes first โ€” you are taking it for confidence, not judgement. Blocks of 20, offline. Do not read this score as a prediction of anything.

Your real baseline โ€” but only after three things are finished.

  • AMBOSS 200 high-yield concepts, every system. The concepts that keep reappearing.
Then give both forms, in blocks. Whatever 27 and 28 expose is the syllabus for 29 and 30. That is their job โ€” not to score you, but to write your next four weeks for you.

  • 31 in 10 blocks of 20.
  • 32 paired with the Med School Guru Free 120.
  • 33 paired with the real Free 120.
Pairing the last two with a Free 120 is what turns a form into a full exam day. All of it in blocks.

How many blocks โ€” the actual numbers

Every NBME form is 200 questions. At the correct block size that is 10 blocks of 20 questions, 30 minutes each. That is the unit for every form from 26 through 33, and it never changes.

That is the whole reason 32 and 33 get paired with a Free 120. A form on its own is 10 blocks, which is most of a day but not the day. Add a Free 120 and you are sitting 14 blocks — the exact shape of Step 1. An NBME measures knowledge; only a 14-block day trains the stamina and the decision fatigue that actually break people at block nine.

Pacing inside every block, without exception: first pass finished by 23 minutes, leaving 5 to 7 minutes spare for flagged questions and biostats. If you cannot do that, pacing is the problem to fix — not content.

What the numbers mean

Not an indicator. One good form out of three is noise, not readiness. Do not book on the strength of it.

Stop and diagnose. Something structural is wrong โ€” do not push forward and hope. See below.
Consecutively and progressively. Two words doing a lot of work. Three scores of 68, 67, 68 is a plateau. 67, 70, 73 is readiness. The direction matters as much as the number.

Build a real exam day

Combine two forms into one 14-block day. A single NBME is half an exam โ€” it measures knowledge but never trains the thing that breaks people: eight hours of stamina and decision fatigue. Pair two forms into a full 14-block sitting and run it exactly like the real day, breaks included.

If the scores are not moving

Consistently under 65% on 27 or 28? Stop. Do not book a date, do not grind another form, and do not add a fifth resource. Something in the method is broken, and another NBME will only confirm it more expensively.
