You Are Already Studying for Step 1.
Final year MBBS and USMLE Step 1 are not two syllabi. They are the same content in two different languages — and right now you are learning it twice, badly, in both.
Ask any Indian final-year student what's stopping them from starting Step 1 and you get the same answer: no time. Medicine postings, surgery postings, paeds, the university exam, the practicals. There is genuinely no free block of months sitting in your calendar waiting for USMLE.
That answer assumes Step 1 is extra work. It isn't. The pulmonary medicine you're being taught on the wards is the respiratory pathology in First Aid. The antibiotics your professor is drilling you on are Sketchy Micro. The difference isn't the content — it's the format and the vocabulary. Step 1 asks it as a vignette; your university asks it as a viva.

One pass, two exams. You study a system once — micro, then First Aid, then your lecture platform, then Mehlman, Anki and UWorld — and everything you learn goes into one book: First Aid. That single habit is what converts your final year from a Step 1 delay into a Step 1 head start.
Four Stages, Day One to Exam Day
Before any of the detail, get the shape of the thing in your head. There are four stages, and the number people quote — 120 days — is only the first of them.
120 days is time-to-content-complete, not time-to-Step-1. It is the systems phase and nothing else. If you read "120 days" as your exam date and book a slot four months out, you will be sitting Step 1 having never taken an NBME. Exam day is at the end of stage 4 — roughly five to five and a half months from day one, of which the first four months run alongside final year and cost you no separate block of time at all.
11 Systems, About Four Months
The plan is system-wise, not subject-wise. Eleven systems, each run through the same three-phase loop — about ten days a system, plus roughly twenty buffer days for the weeks when college eats your evenings. About four months.
Ten days is the working average, not a quota. Dermatology and Psychiatry both run comfortably under it, which buys back time for the heavier ones, and the twenty buffer days absorb the postings, the practicals and the week your college decides to hold an internal exam. The order is yours. If your college is currently doing cardiology, do cardiovascular. That alignment is free leverage — you get the ward teaching, the professor, the clinical exposure and the Step 1 pass all in the same fortnight.
Foundations Go Just Before You Need Them
The standard advice is to do biochemistry and immunology first, as one giant front-loaded block. That's how students burn their first six weeks on molecular biology, retain none of it, and quit. Foundations only stick when they're immediately used. Learn the lipid pathways and then spend the next eight days applying them to endocrine pathology, and you never have to revise them separately. Learn them in January for a system you'll reach in April and you will simply learn them twice.
Every System Runs the Same Three Phases
This is the engine of the whole plan. Eleven systems, one loop, repeated. You don't redesign your study method every fortnight — you run the same nine or ten days, eleven times.

This is a one-day job. Not a one-day-if-things-go-well job. If your system has twelve to fifteen organisms, you finish twelve to fifteen organisms tonight.
Give one full day to reading and understanding that system in First Aid — page by page, with the First Aid Guide open beside it, so every page gives you what is actually tested, an ELI5 explanation, the best videos for that topic and MCQs on that exact page. Not annotating, not making cards — reading it, and getting it. You need the map in your head before anyone starts narrating over it.

Then open whichever lecture platform you already have — Marrow, DAMS or PrepLadder — and watch the videos covering that system's medicine, surgery and paediatrics. The platform genuinely does not matter; they teach the same syllabus. Use the one you have already paid for. This is your college syllabus, and you were going to watch these anyway.

Do the system's questions while the system is still warm — this is the pass that turns recognition into recall. Cross-reference into First Aid. Not into your Marrow/DAMS/PrepLadder notes, not into a fresh notebook, not into a second app. Every extra lecture point, every clinical pearl, every viva-level detail gets written into the margins of the First Aid page it belongs to.
Do this and you finish the year with one book that simultaneously contains your university syllabus and your Step 1 syllabus — and you revise it once. Skip it and you finish the year with lecture notes for college and First Aid for USMLE, which means you did the year twice and revise it twice.
Respiratory, Day by Day
This is a real plan, run end to end on a real system. Nothing here is illustrative — it's the actual instruction, day by day. Respiratory lands at about ten days — a shorter system finishes sooner, and the shape stays identical.
- Day 1 — Micro. Log in to usmlevault.com → Files → System-Wise Microbiology image. Read the Respiratory column: that's 12–15 organisms. Do all of them from Sketchy Micro. Watch the new 2-minute Sketchy Micro video in the University section (not the 13-minute one) and read the PDF directly below it. Finish tonight.
- Day 2 — First Aid. One full day reading and understanding the respiratory chapter in First Aid. Then watch the "How to Do First Aid Guide" video for respiratory.
- Days 3–4 — Marrow, DAMS or PrepLadder — whichever you have. Pulmonary medicine, pulmonary surgery, pulmonary paediatrics. First Aid stays open on the desk the entire time; every additional point goes into the First Aid page, never into a lecture notebook.
- Day 5 — Mehlman, start to finish. Watch the "How to Do Mehlman Medical Videos" guide in the University section, then complete the entire respiratory playlist in the day. No partial playlists — the last third is where the discriminating detail lives.
- Day 6 — Anki. Watch the Anki explainer video in the University section, then run the Foundation Anki respiratory deck. One day, the whole system's cards.
- Day 7 onwards — UWorld. Respiratory questions on the IMD app. The "How to Do UWorld" video and PDF are both in the University section — watch and read them before your first block, once, and never again. Then it's blocks, on your own, until the system is finished. On respiratory that runs about four days; on a shorter system it's two.
- Track your own numbers — this is what ends the system, not the calendar. Write down your percentage after every block. You are looking for one thing: are your last two blocks above 65%? Above it, the system is finished — move on, even if that lands on day eight. Only two items in that list have a fixed length. Mehlman is one day — the whole system's playlist, start to finish, not spread across three. Anki is the day after it — one day, the whole system's cards. Everything before them is your reading, and everything after them is UWorld.
UWorld is the part that flexes. It runs from the day after Anki until your blocks hold above 65%, and that is a different number of days for every system — four on respiratory, two on dermatology, more on neurology. Do not pad UWorld to reach day ten, and do not cut it short to protect day ten.
Write the ten dates down before you start the system, and tick each one off the night you finish it. The failure mode in a four-month plan is never a dramatic collapse — it's a system quietly running fourteen days instead of ten, three times in a row, and the plan losing a month without anyone noticing. A named date on paper is what makes the slippage visible while it's still one day.
Consolidation, and the First NBME
The systems phase — your 120 days — ends around the four-month mark. Month five is not a new syllabus. It's the ten days where everything you've built gets compressed, and then measured.
- Work the Foundation Anki. All of it, cumulatively, not system-by-system any more — ~2,400 cards at roughly 400 a day is a complete First Aid pass in five to six days.
- Complete First Aid. The whole book — including the margins you've been filling since month one. This is the payoff for the cross-referencing habit: your revision material already exists.

- Watch the NBME videos in the University section. That work takes roughly seven to ten days, which puts your first NBME in the first ten days of month five. Attempt it properly — timed, full length, no pausing, morning if you can. Which form, when, and in blocks or full: the exact NBME order — 26 as a warm-up, 27 as your first real assessment, and 31, 32 and 33 as the three that actually predict your score.
After that first NBME you consolidate and sit Step 1 within four to six weeks. Not four to six months. The NBME tells you exactly where the holes are, and holes close fast when they're specific. The students who drift here are the ones who treat the first NBME as a rehearsal instead of a starting gun. The arc, restated
Now that the detail is in place, here is the whole thing again, in order — because this is the version you should be planning against, not the 120.
- ~120 days — the systems phase. 11 systems, about 10 days each, plus ~20 buffer days. Runs concurrently with final year MBBS.
- ~10 days — consolidation and NBME prep. Foundation Anki, First Aid completed, the NBME prep protocol on usmlevault.com. First 10 days of month five.
- The first NBME. About 10 days after the 120.
- 4–6 weeks — consolidate, then Step 1. Close what the NBME exposed, then sit the exam.
End to end: roughly five to five and a half months from the night you open the microbiology image to the morning you sit Step 1 — and the first four of those months cost you nothing you weren't already spending on your MBBS.
What Not to Do
- Don't keep two sets of notes. Lecture notes for college and First Aid for Step 1 is the exact mistake this method exists to prevent. One book. Always First Aid.

- Don't wait for internship. Final year is the only year where your college syllabus and Step 1 overlap this completely. Internship overlaps with Step 2, not Step 1 — and if the plan is all three exams, they fit inside a year: Step 1, 2 and 3 in under 12 months.
